Choose "New Game" from the main menu and enter the following names for the desired effects

Password - Effect

SAMIWAS - Unlock All Missions
SAMIBE - Start with 99 Lives
SAMIAM - Enemies Dont Attack You

After pausing the game, then highlight the RESUME option and push Select, Start.
