Hold down OPTION and COMMAND while selecting new to start on any level.

When you type in these codes hold control:

hell	full heal
arm	armor heal
godeye	shows everything on map
asd	makes super jump
atheist	god mode
Klatn	freezes enemies
woo	gives you pistols and ammo
freedom	gives you shotguns and ammo
xse	gives you m16 with ammo
axes	gives you acces cards
medic	heals team

Click on the "New Game" button, while pressing "Apple"+"Alt", and you will be able to select your starting level.
