To unlock Patty's Gym Clothes outfit you must save all 16 Ugly Secret Babies, making on per incident. You have to complete the incident in order for the baby icon to appear next to the incident on the stage select screen. You'll receive a message stating you have unlocked the outfit once all 16 babies have been saved.
