Maya only has 2 costumes. To get the Gothic Lolita Costume you have to finish Story Mode once. To get the Princess Peach Costume you have to finish all Mission Mode missions on Nightmare Mode.

Gothic Lolita Costume	Finish the game once.
Princess Peach Costume	Complete all Battle Mode missions on Nightmare Mode.

Successfully complete Story mode under Nightmare difficulty setting.

When accessing the Cursed Pages for the first time, only Four Strange Masks and Haunted Doll will be available. Two more can be unlocked by playing certain ones.

Boy in the Book	Complete all Four Strange Masks levels (endless excluded).
Spirit House	Complete the first level of Boy in the Book.
