Use a text editor to edit the "Icewind.ini" file in the game folder. Change the "Suppress Extra Difficulty Damage=0" line to "Suppress Extra Difficulty Damage=1". This eliminates the 150% and 200% increases to monster damage (and experience points) under the hard and insane difficulty settings.
