To unlock FreQ Mode, all you need to do is finish the Campaign once. The difficulty does not matter. You do not need to complete the bonus songs to unlock this mode.

To unlock Super Difficulty, you must beat all fifteen songs in the Expert Campaign. You must get at lest ten bars in each of the three sections to unlock the bonus songs for those sections.

Complete the following tasks to unlock PlayStation 4 trophy rewards.

Trophy	How to unlock
Accelerated Response (Silver)	Clear two consecutive sequences on opposite sides.
Aggregate Results (Gold)	Get more than 45000 points on the overall leaderboard.
Aggressive Tendencies (Silver)	Clear two consecutive sequences and use Eject, Disrupt, or Cleanse against an opponent in between.
Closure (Gold)	Complete all songs on Super difficulty.
Comprehensive Treatment (Gold)	Complete a full 15-song Campaign on Intermediate difficulty or above.
Emergency Care (Bronze)	Restore a broken streak using Cleanse or Flow.
Fluid Intelligence (Gold)	Complete a 15-song campaign on Expert difficulty or above without failing, retrying, or quitting.
Golden Child (Gold)	Get a triple gold bar rating on three different songs.
Group Therapy (Gold)	Score 4000+ on a song as a team.
Imperturbable (Silver)	Keep your streak and earn 100+ points through Disrupt.
Luminary (Gold)	Get a triple bar rating on all songs.
Overachiever (Silver)	Get 100% Sequences Cleared on a song.
Pathway Restored (Bronze)	Complete a song in any mode.
Super FreQ Out (Silver)	Complete a song in FreQ mode on Super difficulty.
Superhuman (Platinum)	Collect all other trophies.
Synesthete (Gold)	Complete Dalatecht on any difficulty in Campaign mode.
Total Awareness (Gold)	Unlock all songs.
