When you get to the spire and are offered your wish (after all the dialog of course) select a wish and wait for your achievement to show up and then turn off your consul. Then go threw it again another 2 times. Then you will have the achievements. But you can only get 1 reward. So choose wisely on which one you want. Greed - 1 million gold (300 evil) , Family- You get your dog back, and sacrifice - you bring back all the people that died in the spire (300 good)
